Insurance
We accept most commercial insurances as well as workers compensation. Contact our business office if you have questions about your insurance coverage.
Please refer to the Patient Financial Policy for Deductible, Co-Pay and Out of Pocket requirements with our office.
Commercial Insurance:
We strongly recommend you contact your insurance directly to determine what has been met of your deductible and Co-Pay. Depending on your policy, your insurance could leave you with an out of pocket expense for services.
Insurance Referrals:
It is the patient’s responsibility to know the terms of their insurance policy. Please be sure to notify our business office if your insurance coverage requires a referral for visits and/or treatment.
Failure to have the proper approval in place could result in rescheduling your appointment or additional expense to you if services are denied for not having a referral in place at the time of your appointment.
If you are unsure about your insurance requirements, we urge you to call your insurance company directly.
Workers’ Compensation:
In order to treat a work injury, we must have the approval of the Workers’ Compensation carrier in place prior to seeing the patient. It is imperative we have the carrier’s contact name and number to call to insure the proper approval for treatment is in place before a patient is evaluated for a work injury.
